Consent form for Show 2022

By registering to take part in July 9th Performance you are agreeing to:

Committing to attending all rehearsals until the show date
Committing to home practice and extra rehearsals
Agreeing to consent to be filmed and photographed for the purpose of both rehearsals and performance, and for their distribution by Clare Connolly Dance
Rehearsals take place between May 31st and July 10th.
Completion of Payment of the Show participation fee. Fees are paid towards administration costs, costume rental, costume collection and distribution, costume alteration, administration costs, extra rehearsals etc.
